Columbia County Health System is seeking a full-time Population Health Nurse who will facilitate patient access to solutions, quality outreach, goal setting, problem solving, and healthcare navigation. The Population Health Nurse works closely with the Population Health Provider Champion, Palliative Care team, and the rest of the interdisciplinary team. This position coaches to promote health and well-being for a defined population, working with chronic care management, ambulatory palliative care, disease specific programs, and Diabetic education. This position will operate within the clinics, in patient homes, and assisted living facilities.This talented team of patient-centered Providers and support staff works seamlessly to ensure our patients receive answers and a plan of action that works best for them. They are actively seeking team members with the confidence to work independently and as a contributing member of the team. Team members must possess a willingness to listen and the ability to maintain a positive demeanor while communicating clearly with patients, staff, and the public.*What You Will Own and Improve in the Role** Collaborate with providers to manage the health of a population of patients and track preventative measures* Providing education and coaching to members enrolled in disease management programs* Assisting the development of patient-centered care plans with patients' wishes/goals and appropriate interventions* Identifying transitional care gaps and assist patients in accessing appropriate services* Monitoring patient progress toward meeting health goals through documentation in EMR and communicate progress to the Population Health team*What You Will Bring to the Position** Interpersonal skills to work alongside multiple patient and coworker personality types* Ability to work well in a team environment, while having the self-starter motivation to work on your own* Advocacy for quality patient care* An understanding of social determinants to healthcare*To Be Successful*This position requires a current Washington State Nursing License (RN) and for you to have completed a 2 or 4 year nursing program from an accredited program. We are looking for an individual who has experience working with diverse patients with complex health conditions and medication management. Demonstration of Motivation Interviewing skills are preferred for this role as you navigate patient health coaching You will have prior experience working in a clinical or home setting with previous roles in Population Health.
