Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel /n Job Description/n American Traveler is seeking a Physical Therapist for a home health position requiring at least 1 year of recent skilled Home Health experience and expert OASIS proficiency. /n /n Responsibilities/n - Work is in a home health setting, visiting patients in their homes - Typical caseload consists of 4-5 patients per day with a 1:1 patient ratio - May visit up to 6 patients per day with a productivity standard of 27-30 points based on territory - Shifts are 8-hour days, Monday through Friday, from 8:00 AM to 4:30 PM - No weekend or on-call required for this assignment - Travel between clients may occasionally require driving up to a 100-mile radius - Proficiency in Homecare Homebase (HCHB) EMR is preferred - Timely communication and documentation are mandatory - Documentation tools will be provided and must be returned upon completion of assignment - System groups client visits to minimize drive and windshield time - First-time travelers are accepted for this position - Locals are accepted for this role - If applying as a travel pair, each traveler will be assigned a separate territory and must have their own transportation - Former permanent employees must be separated for at least 6 months and receive HR approval to be eligible - Requested time off must be submitted at the time of application - Completion of a Hospice/Home Health Checklist is mandatory for consideration /n /n Requirements/n - Current WA PT license or compact PT license required if applicable - Minimum 1 year of recent skilled Home Health PT experience within the past 3 years - Inpatient-only experience will not be considered - Current BLS certification is required - Expertise with OASIS is required - Active driver’s license and personal transportation required /n Benefits/n - High Pay and Bonuses - Medical, Dental and Vision Insurance with Day 1 Options - 401(k) Plan - Weekly Payroll Deposit - Free Online CEUs - Generous Housing Allowance - Travel and Licensure Reimbursements - Non-taxed Per Diem and Subsidy - Traveler Rewards and Discounts /n American Traveler Job ID P-628010.
