REQUIRED FOR BID SUBMISSION - PLEASE READ Documentation of 15 vent
management case logs with the past 24 months. Reports should
clearly highlighted the vent management procedures with dates and
locations for each procedure. We are not able to search through
many pages of case logs to find what we need, it needs to
immediately be clear if they meet this requirement. If details not
provided upon bid submission as listed above, the bid will be
automatically DECLINED and may be excluded from bid submission to
this location in the future. We cannot wait for these items to be
uploaded later. Logs must be uploaded along with CV at the time of
presentation. (Please be mindful to remove any patient/HIPAA
information) Locums- Walla Walla PLEASE NOTE: This facility has an
open ICU; providers will be required to have privileges for ongoing
vent management – must have case logs to reflect 15 cases in the
past 24 months to obtain these privileges. Evidence of these case
logs needs to be included with the presentation, we will not clear
any providers without the relevant case logs attached. Looking for
Hospitalist coverage- 2 providers for day shifts, Tuesday to
Tuesday (7am-7pm), needing coverage for 1-2 weeks a month. Starting
Mid-April/Beginning of May, depending on completed credentialing
