Equipment : PREFERRED: Draeger Tier3 Travel Compliance Compliance
document deadline is Monday prior to start. Last four of SSN RTO
must be listed at time of submittal, post submission RTO will NOT
be approved MUST request all RTO / specific scheduling requests
up-front during submittal. Post submittal/offer RTO may not be
considered or granted. Inflexibility beyond contracted schedule
and/or RTO may lead to termination of contract and system DNR. Max
of one job per submittal Valid WA or Compact RN license (required
at time of submission) Valid Allied license in hand (please ask AM
if unsure) 50 mile local radius, Locals are accepted at a lower
rate All travelers should be prepared to float within the facility
Return Providence Staff must wait 3 months before submitting as

permanent employee, on a travel contract, or as registry/per-diem?
If yes, please include location and dates worked. REFERENCES -
Minimum of 2 supervisory references covering 1 year (6 months for
CNA s) of employment total within the last three years within a
comparable specialty/setting that they are booked. This can include
Charge RNs and Leads. Modules: Modules are non-billable, average 16
hours spent. Over 20 hours requires PM approval Modules are
completed pre-start, and annually Time spent on modules is
self-reported, completion is recorded in Workramp transcripts
