Seeking a full-time Licensed Clinical Social Worker to join rural health clinic in John Day, Oregon.

Details:

• This Social Worker position will be a flexible position to assist with patient and resident needs throughout the Hospital District
• The Hospital District is comprised of four locations
• The Social Worker will work closely with the Hospital Inpatient Case Manager, the Hospice Social Worker and the Care Center Resident Care Manager to meet patient and resident needs
• Participate in performance improvement and continuous quality improvement activities
• Experience in critical access hospital setting, long term care, or Hospice is preferred
